It really throws off the speedometer, and now the odometer is also off. The odometer is usually spot on with the stock gearing. It hasn't affected my mileage at all, if anything it's better than with the stock gearing. Running 70-75 mph for a couple tankfuls netted me around 50 mpg. There's no way I'd go back to stock.
